2026 ENSEMBLE PROGRAMYARD DUTY VOLUNTEER FORM

As a volunteer at MYO:

1. I understand and acknowledge my role is voluntary and am not entitled to any remuneration for my efforts.
2. I understand and acknowledge that I am not an employee of MYO. It is the express intention of the parties that any such relationship is denied.
3. I will actively participate in any necessary training or inductions related to my role and will adhere to MYO’s policies and procedures, including those contained in the Policies and Procedures Manual, as they apply to volunteers.
4. I agree to undertake my volunteering role in a safe manner, following all Occupational Health and Safety guidelines and Child Safety policies and report any accident or injury to myself or another member to an MYO staff member immediately.
5. I agree that I will provide a copy of my Working with Children Check or VIT registration to MYO administration prior to commencing my role, and acknowledge that my ongoing engagement with MYO is conditional on me maintaining these registrations / checks.
6. I understand that all information and identities that I become aware of as a volunteer with MYO is to be held in the strictest confidence, including confidential information and intellectual property, and must not be discussed or divulged to any person or organisation, unless compelled to by law or with the express written authorisation of the CEO of MYO.
7. I understand and acknowledge that MYO has the right to cease my volunteer engagement, cancel, postpone or remove me as a volunteer if deemed necessary as per the Volunteer Engagement Policy. I understand and acknowledge that I also can terminate my engagement at any time by written notice to myo@myo.org.au.
